Physical Therapist (PT) – PRN
The Elms Center | Milford, NH
$55–$60/hour | PRN
Stern Therapy Consultants is hiring a PRN Physical Therapist at The Elms Center in Milford, NH. From assessing a resident's first steps after hospitalization to preparing for a safer transition home, you'll provide skilled nursing rehabilitation centered on each person's mobility needs and recovery goals.
What You'll Do

Evaluate movement, strength, balance, and walking ability to determine appropriate physical therapy services.

Develop measurable goals and individualized treatment plans that reflect residents' prior function and current needs.

Provide therapeutic exercise, gait training, and functional mobility interventions.

Assess assistive device needs and educate residents and caregivers on safe transfers and fall prevention.

Reassess outcomes, adjust plans of care, and direct PTA participation in treatment.

Maintain timely documentation and coordinate with nursing and other clinicians on daily mobility and discharge recommendations.
What We Offer

$55–$60 per hour.

PRN coverage scheduled around your availability and facility needs.

Unlimited referral bonus, with up to $1,000 per successful referral.

Collaboration with the rehabilitation department and support from therapy leadership.
Qualifications
-
Active New Hampshire Physical Therapist license.

Strong evaluation skills and the ability to connect clinical findings with practical treatment goals.

Effective communication, organization, and clinical documentation.

Skilled nursing or post-acute rehabilitation experience is a plus; new graduates are welcome to apply.
