Job Description
The Psychiatric Mental Health Nurse Practitioner (PMHNP) will provide outpatient psychiatric evaluation and medication management services to adults, adolescents, and children at Home Base Collaborative Family Counseling. Home Base Collaborative has been serving New Hampshire communities since 2012 and is currently expanding its services to include psychiatric care. The PMHNP will be responsible for delivering patient-centered care and collaborating with Home Base clinicians to improve patient outcomes and expand the therapeutic reach of the organization.
