Job Description:
\n Shift: Friday, Saturday, Sunday and Monday 8 am to 6 pm
\n Call: Some call may be scheduled, but only on an emergency basis
\n
Requirements:
\n Must have OASIS (Outcome and Assessment Information Set) Experience
\n Home Health and Hospice Experience required
\n Experience traveling from patient to patient for home visits
\n Must be able to manage and adjust to changes in schedule
\n Must have drivers license in good standing
\n BLS required
\n EMR system used is HealthWyse (Careficient)
\n Outpatient care.
