Delivering Solutions that Matter:Provide exceptional physical therapy services to a diverse resident population Perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to maximize residents' mobility, function, and quality of life. Collaborate with residents, families, and the interdisciplinary care team to support rehabilitation goals, coordinate care, and ensure safe, effective discharge planning and referrals.
