Provide expert hospice nursing care in a dynamic home health and inpatient hospice setting in Saint Albans, VT. This contract position offers a unique opportunity to work within a compassionate interdisciplinary team dedicated to end-of-life care across multiple environments including home, skilled nursing facilities, assisted living, and an inpatient respite house.

Responsibilities:

  • Manage a caseload of 12-14 hospice patients in home settings or care for 5-7 patients per shift at the inpatient respite house
  • Deliver comprehensive symptom management and patient education focused on comfort and quality of life
  • Conduct thorough end-of-life care assessments, initiate goals-of-care discussions, and collaborate with patients, families, and healthcare teams
  • Administer medications via varied routes including IV, subcutaneous, oral, and rectal
  • Utilize specialized equipment such as CADD pumps, feeding pumps, suction devices, and respiratory support tools
  • Document patient care accurately in the Net Smart system following thorough orientation and onboarding
  • Adhere to strict protocols and maintain strong communication utilizing agency-issued technology
  • Participate in rotating weekend and weekday shifts, covering a service area including Chittenden and Grand Isle counties as needed

Qualifications and Experience:

  • Current RN licensure (RN-MS preferred)
  • 3 to 5 years of clinical nursing experience with demonstrated hospice or palliative care expertise
  • BLS certification required
  • Skilled in pain management, symptom control, and end-of-life care planning
  • Strong critical thinking and communication skills to educate patients and caregivers
  • Comfortable working independently and managing complex care scenarios
  • Willingness to float between home hospice and inpatient settings as required
