Job DescriptionPosition Overview

The RN Case Managers for both inpatient and outpatient settings play a vital role in our organization, facilitating the transition of patients from hospital care to home or other facilities based on individual needs. These clinical professionals conduct thorough assessments to evaluate patient requirements and available insurance options. Specifically, the Cardiology RN Case Managers collaborate closely with clinical teams to secure essential medical equipment, ensuring a safe and cost-effective transition for patients. This may include items such as intravenous therapies, enteral therapies, durable medical equipment, and dressing supplies. Additionally, RN Case Managers assess the need for services like skilled nursing, home health aides, and personal care attendants, working as part of a multidisciplinary Cardiology team to guarantee that patients have the necessary resources to remain in their communities.

Primary Responsibilities

• Evaluate, plan, organize, and manage daily clinical operations related to care coordination and transitions, addressing the complex health needs of patients and their families.
• Possess a strong understanding of community health services, payer systems, and post-acute care requirements for inpatient families.
• Collaborate effectively with inpatient nursing teams and case managers.
• Integrate regulatory standards, length of stay considerations, resource allocation, funding sources, and capacity management to enhance service delivery.
• Engage actively in hospital, departmental, and multidisciplinary programs, committees, and special projects as needed.
• Commit to a full-time schedule of 40 hours per week, Monday through Friday, from 8:00 AM to 4:30 PM, with rotating holidays and weekends.

Qualifications Education

• Bachelor's degree in nursing is required.
• A Master's degree in a related healthcare field is preferred.

Experience

• At least 5 years of nursing experience is required, with a preference for pediatrics.
• Experience in Case Management, particularly in Discharge Planning and/or Utilization Management, is preferred.

Licensure and Certifications

• Must hold a current Massachusetts Registered Nurse (RN) license.
• Certification as a Certified Case Manager is preferred; if not currently certified, a willingness to obtain this within the first two years of employment is required.
