About the Role:
Solara Hospice LLC is seeking a compassionate and dedicated Hospice Chaplain to join our care team in Waltham, MA. In this meaningful role, you will provide spiritual and emotional support to patients and their families during life's most profound moments. Responsibilities:

  • Provide non-denominational spiritual care and emotional support to hospice patients and their families
  • Conduct spiritual assessments and develop individualized care plans in collaboration with the interdisciplinary team
  • Facilitate religious rituals, prayer, and end-of-life ceremonies according to patient and family wishes
  • Offer bereavement support and grief counseling to families before and after the loss of a loved one
  • Collaborate with social workers, nurses, and physicians to ensure holistic, patient-centered care
  • Maintain accurate and timely documentation of chaplaincy visits and spiritual care plans
  • Coordinate with community clergy and faith leaders to support patients' spiritual needs

Requirements:

  • Master's degree in Divinity, Theology, Pastoral Care, or a related field preferred
  • Board Certified Chaplain (BCC) credential or eligibility through APC, NACC, or equivalent organization preferred
  • Prior experience in hospice, palliative care, or healthcare chaplaincy strongly preferred
  • Ability to provide interfaith and non-denominational spiritual care across diverse populations
  • Strong interpersonal and active listening skills with a calm, compassionate demeanor
  • Valid driver's license and reliable transportation for patient home visits
  • Proficiency in electronic health records (EHR) documentation
