Neuro, Cardiac, or vascular surgery needed.
IR experience: diagnostic and interventional procedures including and not limited to angiography, embolization, venous access, venography, myelography, abdominal interventions, paracentesis, thoracentesis, biopsy, TIPS, dialysis access/fistula work, Y90/SIRT experience.

ARRT certification required.
Primary Source Verification of professional license and/or certification, as required for the position, within 30 days of start date and updated upon expiration.
Must have documentation of BLS certification (AHA, Red Cross, CHSF or Military).

Fit Test (N95) within 365 days to start date and updated annually; no exemptions allowed.
Respirator Medical Clearance (RMC) Attestation required if fit test is required for skill.
Documentation of MMR vaccination or immunity required.
TB Test documentation within 3 months of start date and updated if >6 month break.
Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
Criminal Background Check within 45 days prior to start date and updated if >6 month break.
Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break.
Resume including past 7 years of work experience required for submission.
Two references within 4 years of submission required for submission.
Skills Checklist must be completed within 1 year of submission.
Confirmation of Assignment/POI Form required for all initial assignments and extensions.
Education Verification through background check company required.
SSN Trace must be run with Criminal check.
National Sex Offender Check run within 30 days of start and updated annually.
Exclusion Checks run within 30 days of start and annually.
Candidate Submission Coversheet required for submission to initial assignment and new facility.
Beeline Attestation completed by all new applicants.
