Requirements to Apply
\n
• Bachelor’s degree required
\n
• At least 2 years of recent experience working with elderly or disabled persons
\n
• Strong communication, organizational, and people skills
\n
• Bilingual in English and Spanish
\n
• Ability to work independently, manage member files, and use Microsoft Word/computer programs
\n
\n
Care Manager, full-time role supporting elderly and disabled GAFC members through assessments, care planning, case management, and coordinated services.
\n
\n
Provide psychosocial support, conduct member assessments, coordinate care plans, supervise caregivers, complete home visits, maintain documentation, and connect members with community resources. Work closely with the Registered Nurse and Program Director to address member needs, changes in condition, safety concerns, and service coordination.
