Physical Therapist – Rehabilitation Services
Location: Hyannis, MA
Type: Contract | 13 Weeks
Pay: $62.00/hr – $71.00/hr
Shift & Schedule: Day Shift | Monday–Friday | 40 Hours/Week
Position Overview
We are seeking a compassionate and skilled Physical Therapist to join a dynamic Rehabilitation Services team. The Physical Therapist will evaluate, develop, and implement individualized treatment plans for adult and pediatric patients with the goal of restoring function, improving mobility, reducing pain, and maximizing independence.
The ideal candidate will possess strong clinical assessment skills, excellent communication abilities, and a commitment to providing exceptional patient-centered care in a collaborative healthcare environment.
Responsibilities
• Evaluate patients and establish individualized treatment plans and measurable goals.
• Perform comprehensive physical therapy assessments including strength, mobility, balance, and functional evaluations.
• Develop and implement evidence-based rehabilitation programs.
• Monitor patient progress and modify treatment plans as needed.
• Provide patient and family education regarding treatment plans, exercises, and ongoing care.
• Document evaluations, treatment plans, progress notes, and discharge summaries accurately and timely.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams regarding patient care and discharge planning.
• Supervise and mentor Physical Therapist Assistants, rehabilitation aides, and students.
• Assist with orientation and training of new staff members as needed.
• Maintain compliance with departmental policies, infection prevention standards, and patient safety protocols.
• Participate in departmental meetings, continuing education, and quality improvement initiatives.
Required Qualifications
• Bachelor's Degree in Physical Therapy from an accredited Physical Therapy program.
• Active Massachusetts Physical Therapist License.
• Current BLS Certification.
• Experience evaluating and treating adult and pediatric patients.
• Strong rehabilitation assessment and treatment skills.
• Excellent communication and documentation abilities.
Preferred Qualifications
• Acute Care experience.
• Inpatient Rehabilitation experience.
• Outpatient Rehabilitation experience.
• Hospital-based Physical Therapy experience.
• Experience supervising Physical Therapist Assistants, aides, or students.
