Licensed Practical Nurse, Long Term Care | Rutland, VT | start 11/02/2026 for 8 weeks | 40 guaranteed hours per week | 5x8 days (06:00–14:30) | Skilled Nursing Facility travel assignment. Local only; PCC-trained required.

Responsibilities
- Administer medications and treatments per physician orders and facility policy
- Conduct regular resident assessments; document changes in condition
- Monitor vital signs, identify deviations, and escalate concerns to the supervising nurse
- Assist residents with activities of daily living; promote mobility and independence
- Maintain accurate nursing documentation; participate in care planning and shift handoffs
- Collaborate with the interdisciplinary team; follow PCC-trained protocols and infection control standards; respond to emergencies per protocol
Requirements
- Current Licensed Practical Nurse license in good standing; ability to practice in Vermont
- PCC-trained; local candidates only
- CPR/BLS certification
- Education: graduate of an approved practical nursing program
- Experience: LTC or skilled nursing experience preferred
- Ability to work in a skilled nursing environment and meet the defined shift requirements
