Job DescriptionJob Title: Travel Registered Nurse – HospiceLocation: South Dennis, MADuration: 13 WeeksSchedule: Day/Evening Shift | 8:00 AM – 4:00 PM (Weekday, Weekend & Holiday Rotation)Job Overview:
We are seeking a compassionate and experienced Travel Registered Nurse (RN) – Hospice to provide high-quality end-of-life care to patients in home and facility settings. This role requires strong clinical judgment, effective communication, and the ability to manage a patient caseload independently while collaborating with an interdisciplinary team.
Key Responsibilities:
- Provide compassionate hospice nursing care to patients in homes and care facilities
- Conduct comprehensive patient assessments and respond to changes in condition
- Manage and coordinate hospice patient caseloads and individualized care plans
- Participate in interdisciplinary team meetings and case conferences
- Communicate effectively with physicians, patients, caregivers, and healthcare teams
- Complete patient admissions, charting, and documentation accurately and on time
- Obtain and clarify physician orders for patient care
- Educate patients and families on hospice philosophy, care plans, and services
- Ensure compliance with hospice regulations, nursing standards, and internal policies
- Supervise Hospice Aides every 14 days per regulatory guidelines
- Support orientation and mentoring of new staff as needed
Required Qualifications:
- Active Registered Nurse (RN) license – Massachusetts
- Minimum 2 years of Hospice RN Case Management experience
- Strong clinical assessment and critical thinking skills
- Excellent verbal and written communication skills
- Ability to work independently and manage schedules efficiently
- Successful completion of pre-employment medication competency test
Preferred Qualifications:
- Prior Hospice Home Care experience
- IV Certification
- BSN (Bachelor of Science in Nursing) preferred
- Proficiency with electronic medical records (EMR) systems
Work Environment:
- Home health and hospice care settings
- Travel between patient locations required
- Collaborative, patient-centered care environment
