Job Title: Registered Nurse (RN) – Home Health
Location: South Dennis, MA
Type: Contract (13 Weeks)
Shift & Schedule: Monday – Friday | 40 Hours Per Week
Pay: Up to $2,800 Weekly (Based on Eligibility and Experience)

Position Overview:

Navitas Healthcare, LLC is seeking a Registered Nurse (RN) – Home Health for an exciting Contract job in South Dennis, MA.

Responsibilities

• Perform comprehensive nursing assessments, including physiological, psychosocial, environmental, and health history evaluations.

• Develop, implement, and revise individualized patient care plans based on assessment findings.

• Evaluate patient needs and coordinate appropriate interdisciplinary services.

• Participate in patient case conferences and collaborate with healthcare providers regarding care plans.

• Provide skilled nursing care and document all services accurately and timely.

• Supervise and instruct Home Health Aides in accordance with established care plans.

• Notify physicians and care team members of significant changes in patient conditions.

• Obtain and clarify physician orders for all patient care services.

• Coordinate patient discharges and complete all required discharge documentation.

• Refer patients to appropriate community resources when needed.

• Maintain compliance with agency policies, clinical standards, and regulatory requirements.

Required Qualifications

• Graduate of an accredited School of Nursing.

• Active Massachusetts RN License.

• CPR Certification.

• Additional specialty-related credentials as required.

• Minimum 1 year of nursing experience.

OR

• 6 months of nursing experience with a BSN.

• Successful completion of VNA pre-hire medication test.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).

• Home Health experience.

• IV Therapy experience.

Additional Requirements

• Travelers must secure housing on Cape Cod (minimum Mid-Cape area).
