CARE CONTINUUM COORDINATOR SCHEDULE

CARE CONTINUUM COORDINATOR POSITION SUMMARY


The Care Continuum Coordinator coordinates, conducts, and implements discharge planning for patients in the MAT Program. Maintain effective communication with facility staff and community resources as well as maintain accountability for services provided.


CARE CONTINUUM COORDINATOR ESSENTIAL FUNCTIONS

  • Responsible for discharge planning for patients receiving MAT services in accordance with the policy and procedures.
  • Responsible for creating and maintaining relationships with community agencies providing services to people with SUD.
  • Coordinate with child protective services (DCF) related to housing and resources.
  • Responsible for abiding by security regulations, including limits of confidentiality of patient information requirements of the facility.
  • Identify release planning needs of patients receiving MAT services.
  • Assist patients in making appointments with community resource agencies for continuation of services.
  • Communicate with agencies serving those receiving MAT services to develop options for residential placements.
  • Communicate and maintain positive working relationships with appropriate Unit Team Staff, Institutional Parole Officers, Re-Entry Case Management, Mental Health Department, or other facility staff also assigned to release planning activities.
  • Document services provided to the inmate in the health record, including contacts with community providers or parole staff after patients release from custody.
  • If required by contract, maintain tracking logs of any follow-up contacts provided after patients are released from the facility.
  • Develop resource list for a variety of housing options and work with patients to identify appropriate housing.
  • Facilitate applications for transportation as needed.
  • Track patient release dates and upcoming court appearances.
  • Attend treatment team meetings to enhance release planning activities and maintain a team approach.
  • Participate in staff meetings and in-service training as provided for MAT services.
  • Attend community based meetings relevant to release planning as directed by the Clinical Supervisor of Mental Health Director.
  • Compliance with employee standards of the facility. Compliance including but not limited to, the maintaining of a professional working environment and personal appearance consistent with professional responsibility, development of positive working relationships, and timely notifications of supervisory personnel of absences from the institution.
  • Maintain proficiency with all software required for the position, and more specifically, clinical documentation in the designated electronic health record system.
  • Providing education/ensuring all MAT patients are provided access to Naloxone at the time of release whenever possible.
  • Perform other duties as assigned.

CARE CONTINUUM COORDINATOR MINIMUM REQUIREMENTS

  • Bachelor’s degree in social work or closely related field from an accredited college or university.
  • Previous experience in developing successful transition plans involving community resources for persons with Substance Use Disorder.
  • Must also have experience in case management in a Human Service agency.
  • Must maintain CPR certification, and any other certifications required by contract.
  • Satisfactory background investigation report.
