This is an onsite 20 hours per week part-time position.


Position Summary:

The Community Health Worker/Patient Navigator (CHW) will provide patient navigation and care coordination services for individuals undergoing or eligible for Hepatitis treatment. Support patients in engagement and retention in care, including appointment scheduling, follow-up, and coordination with clinical and support services. Assist patients with treatment adherence through education, reminders, and supportive coaching using a trauma-informed and patient-centered approach. Coordinate laboratory testing, referrals, and follow-up appointments relate to Hepatitis care. Assist with prior authorizations and care coordination activities related to Hepatitis treatment, in collaboration with clinical staff and pharmacy partners. Maintain regular contact with patients through outreach efforts to re-engage individuals who are out of care or overdue for services.

The CHW will support the care team working collaboratively with interdisciplinary care team members, including providers, nurses, case managers, pharmacists, and external partners. Support program goals related to viral suppression, engagement in care, and improved health outcomes. The CHW will document all services as required and will assist in meeting all program requirements.

Experience and Education Required:

  • Minimum of two years relevant work or college-level internship experience required.
  • Data entry skills and proficiency with MS Office including Excel and Outlook.
  • Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners.
  • High school diploma or equivalent required; associate degree or relevant postsecondary education preferred.
  • Preferred candidate will have Community Health Worker or Medical Interpreter certificate of completion.
  • Have organizational skills and the ability to be able to work independently.

Professional Licensure/Special Skills and Certification Required:

  • Massachusetts Community Health Worker Certification preferred. Community Health Worker Core Training required within first year of employment.
  • Must be bilingual in English and Spanish, or other language appropriate to the health center’s patient population.
  • Local travel required for home visits to patients.
  • Flexible evening work, weekend hours required to meet FHCW community or patient needs.
