- MGB Medication Exam with 80% passing within 365 days
- Documentation of MMR vaccination or immunity required
- Hepatitis B vaccination or immunity strongly preferred
- Influenza Vaccine required during flu season (Sept 15 - April 30) with no exemptions
- TB Test documentation within 3 months of start date and updated if >6 month break
- Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- Tdap vaccine within 10 years preferred
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
- MGB Massachusetts CORI check within 30 days prior to start date and updated if >6 month break
- MGB Resume including past 7 years of work experience required for submission
- MGB References: 2 recent references required, minimum 1 Supervisor or Above in skillset
- MGB Skills Checklist required for submission
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
- Positive PPD with negative chest X-ray documentation if applicable (optional)
- MGB Education Verification required for onboarding
- MGB SSN Trace required for onboarding
- MGB National Sex Offender Check required within 30 days of start and updated annually
- MGB Exclusion Checks required within 30 days of start and annually
- MGB TB Symptom Survey if Positive PPD (optional)
