About the Role
We are currently in search of a dedicated Part-Time RN/LPN to join our evening shift team (3 PM - 11 PM). In this role, you will deliver direct care to our residents in a collaborative environment, ensuring that their needs are prioritized in our Skilled Nursing facility.

Key Responsibilities
- Carry out professional duties in line with facility policies, state regulations, and standards of practice, including the Nurse Practice Act, OBRA, and HIPAA.
- Work collaboratively with an interdisciplinary team to develop and update personalized care plans for residents, participating in care plan meetings.
- Implement care protocols as needed, focusing on areas such as fall prevention, safety, behavior management, weight management, skin integrity, bowel/bladder management, infection control, and pain management.
- Assess and document any changes in residents' physical or mental health, reporting findings to the primary care physician and treatment team to support informed decision-making.
- Provide leadership to direct caregivers, assigning daily tasks and supervising care through observation and evaluation.
- Foster positive relationships with team members, residents, and their families to encourage open communication.
- Administer and document medications and treatments according to professional standards and nursing policies, ensuring proper inventory management of narcotics.
- Promote and maintain a positive culture for both team members and residents.
Qualifications
- A minimum of a Nursing Degree or Diploma from an accredited institution.
- Experience in long-term care and specialized knowledge in elderly and rehabilitative nursing is preferred.
- Must possess a valid RN or LPN license in the State of New Hampshire. Certification in intravenous administration is a plus. Continuing education units (CEUs) must be maintained annually, and CPR certification is required.
