VNA Care is hiring an LPN Care Coordinator for our Dorchester/ Boston Teams. This is an interim position for approximately 6 months. Once this role expires, an LPN would have the opportunity to apply for other LPN roles within the company.
Position Summary: Assists Clinical Manager with the activity of clinical team members and assumes responsibility to meet VNA Care and clinical department goals and VNA Care core values.
Reports to: Clinical Manager
Schedule: Monday -Friday 7:30a- 4:00p. Office location is 150 Mt Vernon Street, Dorchester.
Functions & Responsibilities:
· Communicates findings, issues and problems regarding compliance, documentation and clinical practice to Clinical Manager as appropriate using effective communication skills.
· Monitors capacity levels to ensure appropriate resources for client needs; determines and adjusts capacity management/collaboration with schedulers as needed. Triages client referral sources and staffing needs and works to improve the client experience through consistency in capacity patterns.
· Assists in obtaining physician orders to expedite client care and serves as a resource to providers to facilitate processing new orders.
· Shares appropriate information in ways that are clear, understandable and meet customer needs in order to establish mutual trust and respect.
· Promotes and facilitates understanding of the fiscal impact of all clinical practices and documentation.
· Provides support in homecare/homebase workflow as needed.
· Maintains knowledge of third party reimbursement requirements and regulations and applies knowledge effectively to the review process.
· Provides direct service excellence to key providers and referral groups, building and enriching relationships, identifying needs, problem solving and meeting or exceeding expectations of external customers.
· Participates in the development of strategies to facilitate the ideal communication and collaboration between VNA Care, key provider groups and referral sources, working collaboratively within the pod and with other Agency departments.
· Demonstrates initiative and takes action to satisfy customers and takes responsibility for ensuring a positive experience for outcomes.
· Shares firsthand customer information with appropriate groups that can be used for service design, evaluation and improvement.
· Helps resolve problems that affect service, efficiency, productivity and customer satisfaction and participates in service recovery as needed.
· Identifies when a situation or problem goes beyond ability to resolve and escalates to the appropriate Agency staff.
· Participates in activities that enhance self-competence and professional growth.
· Ensures all documentation supports the clinical acuity, functional ability of the patient and the reimbursement needed to support the patient service care needs.
· Oversees and executes expectations and standards of excellence that lead the organization in proactive and continuous quality improvement.
· Upholds the compliance objectives and policies and procedures of the Foundation and Subsidiaries.
· Performs other related duties of a similar nature and complexity as directed.
Skills and Knowledge:
Education
· Graduate of a certified Licensed Practical Nursing program.
· Licensed to practice in Massachusetts.
Experience
· Minimum of 2 years clinical home health care experience, preferred.
