Shoes must be worn at all times in the production area. Stockings or socks are required. The heel of a shoe must not be pointed or higher than 2 inches. Sandals, clogs, slippers, open toe, or open heel shoes are not allowed. Shoes with straps or weave where toes can be seen are not allowed. Changing shoes in the production area, other than a gowning room, is not allowed. Torsos, upper arms, and legs must be covered. Pants (above the ankle), skirts (above the ankle), skirts with slits, short shirts (above the navel), and sleeveless tops are not allowed. Skirts at the ankle are allowed, but full-length stockings, leggings, or pants must be worn under them. Nail polish, acrylic, artificial nails, or similar nail coatings are not allowed. Jewelry or body piercings that can pierce gloves or cannot be covered under gowning materials will not be allowed in the production area. Makeup shall not be worn. Lip balm in stick form is acceptable, when applied outside of production areas and prior to hand washing. Scented personal use products shall not be worn, except deodorant.  The Clinical Care Transitions Specialist - RN works with the intake department to complete timely informationals, hospice clinically eligible assessments, and admissions with a primary focus on facilities and hospital settings. This role would provide formal and informal hospice training to facility staff from basic Hospice 101 to Pain and Symptom Management. Partnership with the Hospice Medical Director and Clinical Leadership will be critical for eligibility determination. 
 
In collaboration with the hospice team, the Specialist ensures the timely, coordinated care of patients transitioning from home to a facility or hospital to home and other care settings. The position is focused on ensuring seamless coordination of care with referral sources and the hospice team.

Support increasing access to hospice by collaborating internally with HCS team members and with HCS referral sources. Responds to requests for informationals timely which lead to assessments for clinical eligibility and admission, working closely with the hospice provider and clinical leadership.

What You’ll Do

\n\n
As a City Year AmeriCorps member, your service will focus on coaching and

\n\n
mentoring elementary, middle and/or high school students. You will help these

\n\n
students earn academic success, improve their attendance, and develop positive

\n\n
behaviors at school. You will plan and monitor individual student progress and

\n\n
work with teachers on whole class instruction, breakout spaces and other in-class

\n\n
student assistance.

Role and Responsibilities

\n\n
Experience in all these tasks is not necessary. We provide 2-4 weeks of training at

\n\n
the beginning of the year to support you. Plus, you will work with partner teachers

\n\n
and school staff every step of the way.

Support academic coaching and interpersonal development for students:

\n\n
· Work with identified students 1:1 and in small groups

\n\n
· Develop literacy and/or math skills varying learning activities by student level,

\n\n
answering student questions and encouraging assignment completion

\n\n
· Create and implement a behavior management system for students

\n\n
· Develop interpersonal skills throughout their day, using proven strategies of

\n\n
learning, practicing, reflection and repetition

\n\n
· Collaborate with teachers and team members to collect, record, analyze and

\n\n
monitor individual student progress, participating in facilitated reviews of student

\n\n
data collected and entered into City Year’s student data system· Create an engaging, safe and healthy classroom space in collaboration with the teacher

Support Whole School initiatives to increase engagement with students and

\n\n
families:

\n\n
· Support and/or conduct extended day and virtual programming for students

\n\n
before, during and after school including, but not limited to, academic enrichment

and intervention, homework support, recreation and student clubs, service

learning, and leadership development

· Create, support and/or co-lead events or programs that recognize and appreciate
