Summary: The Direct Support Professional 3 will support and assist in Level 3 residential programs. Staff will help individuals who need daily intervention, supervision, and skills training in activities of daily living while managing within a home environment and in the community. <li>Transport and accompany individuals as needed to and from medical, recreational, and other activities outside the program.</li>
<li>Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF, and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Assist individuals with accessing resources and recreational opportunities in the community.</li>
<li>Administer medications according to MAP regulations, as applicable.</li>

Job DescriptionDescription The Residential Services Coordinator’s role is to work closely with the ownership and property management team to bring forth and address the needs and the dilemmas of the residents. The Residential Services Coordinator provides support, intervention and lease education to the residents. The RSC also serves the residents by providing informal counseling, referrals to individual residents and access to other social services. The RSC uses creative and innovative methods to connect with the residents. In the effort to support the residents, the RSC administers on-site programs such as health safety sessions, educational programs, job training/preparation development programs for the resident. The Residential Services Coordinator's role is not only to maintain the standard of life for the residents but to improve their quality of life. The Residential Services Coordinator strives to bridge the relationship between residents, the ownership and property management team and the neighborhood to ensure a healthy housing community. Other Responsibilities include: Follow up on referrals, conduct meetings with residents, refer residents to services, provide lease education and keep the Property Manager informed of significant developments related to lease violations and ongoing resident issues.
