Report resident’s intake to Team Leaders. Offer the resident nourishments and/or distribute nourishments from the dietary department. Check fall precautions: bed monitor, mats, and personal alarms. Check and clean dirty wheelchairs after each meal or after incontinent episode. Check and clean tub and shower room. Greet newly admitted residents upon admission, escort to room as necessary, receive paperwork from referring agency, and inform Unit Manager/Charge Nurse of arrival. <li>Support and assist individuals with all activities of daily living during the morning routine.</li>
<li>Assist individuals during the overnight shift who may wake up or require assistance.</li>
<li>Provide services to individuals according to, DDS, DMH, MassHealth, HUD, DPH, CARF, all applicable regulatory standards, and Bridgewell policies and procedures.</li>
<li>Maintain daily and monthly progress notes, shift checklists, and other program documentation according to DDS, DMH, and all applicable regulatory standards.</li>
<li>Complete household cleaning duties as assigned.</li>
<li>Maintain safety of all individuals while on shift including implementing emergency policies as needed.</li>
<li>Administer medications according to MAP regulations.</li>
<li>Maintain core training certifications including CPR/FA, OSHA, CPI, PABC, and MAP if applicable.</li>
<li>Maintain additional mandated training and certifications as required by the program.</li>
<li>Transport individuals as needed.</li>
<li>Other duties as assigned.</li>

Required Education/Experience:
<ol>
<li>Must have a high school diploma or GED. Associates or DSP Certification preferred.</li>
<li>A minimum of 1 year of experience working with people with developmental or psychiatric disabilities who have significant medical and/or behavioral needs is necessary.</li>
<li>A bachelor's degree in a related field is preferred.</li>
</ol>
Required Skills/Knowledge:

<li>Excellent written and oral communication skills so that the writing of required documents is clear and articulate.</li>
<li>Excellent organizational and problem-solving skills.</li>
<li>Valid driver's license and one year of driving experience.</li>

Summary: The Direct Support Professional 2 will provide support and assistance in Level 2 residential programs. Staff will provide support to individuals who need daily intervention, supervision and skills training in activities of daily living while managing within a home environment and in the community. Individuals may require moderate physical assistance or accommodation. Staff will receive and must pass specialized training in order to provide enhanced supervision or interventions designed to address multiple disabilities which may include behavioral supports or enhanced medical care.
Schedule: Sunday 9:00 AM - 2:00 PM, Thursday 4:00 PM - 9:00 PM, Saturday 5:00 PM - 10:00 PM (15 hours)
Essential Duties and Responsibilities:

<li>Create opportunities and provide support to individuals to make decisions in order to develop their skills and foster independence.</li>

<li>Provide support and assist individuals with all medical, clinical and financial needs.</li>
<li>Transport and accompany individuals as needed to and from medical, recreational and other activities outside the program.</li>
<li>Maintain required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards as well as Bridgewell policies and procedures.</li>
<li>Assist individuals with accessing resources and recreational opportunities in the community.</li>
<li>Administer medications according to MAP regulations, as applicable.</li>
<li>Maintain core training certifications including FA/CPR, PABC and MAP.</li>
<li>Maintain other additional required training and certifications as required by the program.</li>
<li>Develop positive relationships with guardians, family members and other stakeholders.</li>
