Community Bridges is looking for a part-time Behavioral Direct Support Professional in Concord, NH! 
 
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
 
The DSP - Behavioral responsibility is to be a community connector and will be responsible to do community mapping with the individual to promote social inclusion. The DSP - Behavioral will explore new opportunities for individuals to whom they provide supports and will assist with growth and unity within the community. Promoting abilities not disabilities and to develop a bond between individual and community, family, friends and significant other in their lives. More importantly, to identify those assets that most likely lead to social inclusions for a specific individual whose gifts and interests have been identified.
 
Qualifications:
Education: High School Diploma or GED required.
Experience: Experience in the human service or related field with persons experiencing disabilities desirable. 
Training: MOAB Day 1, MANDT, or formal de-escalation training.
Reliable transportation, a valid driver's license, and proof of automobile insurance.
 
Compensation and Benefits:
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

<li>Assist individuals in establishing, maintaining, and presenting a positive image. This may involve such activities as assisting in the bathroom, bathing, showering, and dental hygiene, etc.</li>
<li>Maintain a calm demeanor and respond quickly and appropriately to aggressive or verbal outbursts. Scan room and individual for potential danger.</li>
<li>Assist in developing and individualizing the yearly Person Centered Planning by providing information for assessments and by developing implementation strategies for goals/objectives and methods for monitoring the delivery and quality of supports to the individual.</li>
<li>Complete documentation requirements as specified by the Individual Service Plan to include the scheduled data collection and monthly progress notes to show an increase or decrease in behaviors to continually support individuals as they grow and reach goals.</li>
<li>Assist in the orientation/training of new staff, which may include introduction to an individual's support persons and to an individual's ISP, strategies, dreams and goals, and treatment plan. </li>
<li>Distribute medication in accordance with He-m1201 as applicable.</li>
<li>Report any safety, health or human rights concern immediately to the Supervisor. </li>

Job Summary
The Resident Care Director (RCD) is responsible for the overall safe operation of the resident services department. The Resident Care Director plans, implements and evaluates the safe operation of the resident care department, staff, and residents of the community. The RCD will continuously evaluate the health functioning of all residents, and design and revises the care plan for each resident as necessary. This position ensures compliance with all applicable regulations and company policies and procedures. 
 
Duties and Responsibilities
The following essential functions are the fundamental job duties of the position to be completed with or without appropriate reasonable accommodation.

<li>Identify and assess the clinical status of potential residents including complete nursing assessment and consultation with current PCP prior to admission.</li>

EPIC, a part of Triangle, was founded in 2011 with the mission of preparing young people with disabilities to be actively engaged community leaders through education, leadership development and community service. About the Job: Triangle is looking for a dependable individual committed to assisting youth with disabilities to fulfill their leadership potential in the South Boston Areas. The qualified candidate will assist youth with disabilities in providing personal care and needed accommodations to ensure they are able to participate in EPIC programming, in addition to supporting staff with the execution of programming. Essential Duties and Responsibilities: • Assist EPIC youth with personal care routines as needed (toileting, eating, etc.) • Support youth by providing needed accommodations to ensure their participation in EPIC programming • Work with EPIC staff to assist in preparing for programming to make sure the needs of all of EPIC’s young leaders are being met • Supervise youth during workshops, volunteer opportunities, and recreational activities • Represents Triangle and EPIC to family members, community organizations, and partners • Provide Program Participants with instruction in performing community service projects and other programming • Ensure that all personal care assistance provide to EPIC youth fits into EPIC’s self-advocacy model by ensuring that all assistance is youth led Additional Duties and Responsibilities: • Maintain proper care, use, inventory and storage of supplies and equipment • Identify and assist in curriculum development and execution • Participate in all required training • Intervene in minor behavioral issues • Participate in team meetings • Assist in reaching programmatic goals set by EPIC Executive Director • Other duties as assigned Please note: This position requires traveling to different partners. Candidate must have a valid driver’s license or dependable transportation. Candidate must pass a pre-employment CORI and Driving Record Check. Education and Experience: A minimum of 2 years of experience in human services, that includes experience working with youth with disabilities. Experience assisting people with disabilities with their personal care.
