Under the general supervision and direction of the Manager, Respiratory Care or designee, perform diversified duties in administering and monitoring Respiratory Care Services according to Physician's orders. Services including but not limited to; Chest Physical Therapy (CPT), sputum induction, breathing exercises, medication delivery, fit testing, Tuberculosis (TB) testing, blood sampling and analysis, Cardiopulmonary Resuscitation (CPR), airway management, bronchoscopy assistance, evaluation and assessment, protocol implementation, patient and family education, smoking cessation, non-invasive and invasive mechanical ventilation, airway management, heliox therapy, oxygen therapy, holter monitoring, Electrocardiogram (EKG) testing and Pulmonary Function Testing (PFT).
Qualifications
* Graduate of an approved program for Respiratory Care is required; an Associates degree is preferred. 
* CRT is required, RRT is preferred. 
* Valid license from the Massachusetts Board of Registration in Respiratory Care is required. One (1) year of acute and chronic respiratory care experience is preferred.Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Responsibilities: Under general supervision and medical direction performs respiratory care related procedures. Exercises considerable judgment in accordance with physician's treatment plan and within the scope of departmental policies and procedures. Scope of practice includes patient/family assessment and management, resource management, care facilitation, and discharge planning as required. Assesses resident's condition, administers prescribed therapy and aerosolized medications, observes and records resident's progress and response to therapy modalities including adverse reactions to drugs and therapy. Provides in-service training to residents, family members, and facility staff. Resident population is primarily adult and geriatric. Utilizes knowledge of the principles of growth and development appropriate to the resident's age to ensure that care provided meets the unique, physical and psychological needs of the resident. Involves appropriate parties (i.e. resident's family or facility staff) in the assessment, treatment, and continuing care of the resident as needed. 
Initiates, monitors and maintains life support systems for residents in the skilled nursing facility. 
Performs airway care and maintenance. 
Manages artificial ventilator status as directed by an approved protocol. 
Maintains and troubleshoots mechanical ventilators. 
Participates in cardiopulmonary resuscitation procedures of bag ventilation and closed chest massage. 
Initiates, monitors and documents performance of various respiratory care modalities. 
Performs assessment of residents' cardiopulmonary status utilizing physical assessment and history taking skills, laboratory findings, radiological studies of the chest, pulmonary function testing and professional judgment to formulate a respiratory care plan upon the order of a physician. 
Performs various tasks prior to treating a resident as follows: 
Check physician's orders for completeness. 
Orders to include: diagnosis, ICD -10 code, type and duration of therapy, type and dosage of medication, FI02 and frequency. 
Check resident's chart for history, physical and for pertinent information to determine precautions to be observed at bedside. 
Observe and chart: pulse, respiration, breath sounds (number and quality), and resident's general appearance, including breathing exercises and teaching and training to residents. 
Notify supervisor of all new starts and discontinued residents. 
Administers proper technical respiratory care procedures. Dispenses proper dosage of medication, acquires and adjusts devices to insure optimal results from therapy. This includes mask, mouthpiece, or Trach equipment for resident's comfort and ease of operation, regulates flow and pressure of equipment. Observes resident's respiratory rate and pattern, pulse, breath sounds, reaction to medication, any difficulty with therapy during and after treatment. Chart and communicate to the resident's nurse any pertinent information. Observe thorough and accurate charting technique. 
Inspects and tests equipment, disassembles, cleans, reassembles equipment as required. Maintains adequate supply of necessary equipment. Performs administrative duties and record keeping when required. Disposes of equipment properly. 
Assists in emergency cardio-pulmonary resuscitation as an integral part of the emergency team. Therapist is required to be CPR certified. 
8. Initiates, administers, evaluates response to, and documents all therapy modalities according to departmental procedure and ordered by a physician including: intermittent positive pressure breathing, hand held nebulizer, chest physical therapy modalities, including percussion, vibration, breathing and cough retraining exercises, deep tracheal suctioning, incentive spirometry, sputum induction. 
Recognizes resident's adverse reaction to therapy modalities and takes correct action. 
Documents all resident care services rendered as part of a medical record. 
Accepts and transcribes verbal orders related to respiratory care procedures. 
Sets up, maintains and records log entries on all types of oxygen and respiratory supplies and equipment as required.

Qualifications: A Respiratory Therapist must have a high level of knowledge of respiratory therapy and practice required through completion of a NBRC approved and accredited program and one year of clinical practice experience under medical direction. 
Must have a current Basic Cardiac Life Support certification and current State Licensure for a Respiratory Care Practitioner.

Under the general supervision of the Case Management Manager acts as a patient advocate/Case Manager to SSH&amp;EC clients. An autonomous role that coordinates, negotiates, procures services and resources for, and manages the care of complex patients to facilitate achievement of quality and cost efficient patient outcomes. Looks for opportunities to reduce cost while assuring the highest quality of care is maintained. Applies review criteria to determine medical necessity for admission and continued stay. Job Description
1 - The RN Case Manager is responsible for reviewing the medical record of all observation and inpatient admissions and continued stays to ensure appropriate utilization and delivery of care. 
a - Using Interqual Criteria, physician certification, and payor specific criteria, assists the physician in determining the medical necessity for observation, admission and continued stays. 
b - Identifies cases daily that fail to meet criteria and refers these cases to appropriate manager or physician advisor for secondary review. 
c - Contacts attending physicians daily on cases that lack adequate documentation warranting acute hospitalization and clarifies for them the necessary clinical documentation required to help support medical necessity 
d - Contacts the attending physician to notify him/her of decision to issue notice of non-coverage. Explains UR process and insurance coverage requirements. Obtains physician written concurrence when necessary; e.g., Medicare patients. 
Informs the patient and/or next of kin when insurance coverage must be terminated for the current admission. Issues the termination letter for the Medicare patient 
e - Reinstates insurance coverage when patient condition becomes acute and meets criteria again. Issues reinstatement letter. 
f - Continues review of all patients using criteria and determines need for continued hospitalization based upon third party payor/insurance guidelines. 
g - Provides clinical data/information to contracted third-party payers while patient is hospitalized to ensure continued reimbursement and to avoid reimbursement delays within 24 hours of request.

2 - Plays an essential role in assisting physicians, nursing and staff with accurate determination of a patient's observation status. The RN Case Manager is an important resource in preventing delayed discharges of observation patients. 
a - Identifies and reviews observation patients to determine the correct patient level of care daily prior to 12 PM. 
b - Consults with physicians, nursing, admitting, and outside insurance case managers to determine the appropriate status of patient. Refers the questionable status to internal physician advisor or EHR according to the Departmental Process. 
c - Assumes the role of review coordinator for observation services; reviews medical record for appropriateness of status and level of care, and facilitates the level of care, utilizing InterQual for Observation. 
d - Works with physicians, nursing and staff, patients and families to arrange prompt and safe discharge 
e - RN Case Manager must take telephone orders from physicians changing patient status from observation to inpatient admission. This should be done when monitoring observation status. A call or page should be made to physician if the RN Case Manager believes that this should be an inpatient admission and not wait until the 24 hours are ending before conversion. RN Case Manager must actively monitor patients on observation status and seek to clarify their status as close to the 24-hour benchmark as possible. The RN Case manager must send a concern in a timely fashion to facilitate the patient being put into the correct patient status and to provide timely notification. 
3 - Participates in case finding and pre-admission evaluation screening to assure reimbursement. 
a - Identifies potential transition planning problems in a timely manner to set up services required. 
b - Works with attending physician to move patient through the SSH&amp;EC system and set up appropriate services or referrals; e.g., SNF/VNA/Home Pharmacy 
c - Identifies need for new resources if gaps exist in service continuum, and initiates creative care delivery options. 
4 - The RN Case Manager is responsible for assessing patient acute level of care needs and works to implement and coordinate interventions aimed at facilitating a safe and timely discharge plan to the appropriate sub-acute settings in collaboration with the Case Manager Specialist. 
a - With the Case Manager, work to identify, and prioritize workflow through identification of patient specific, department needs and or unit based needs. 
b - Executes and implements a safe and effective discharge plan based on the case management assessment in accordance with the Conditions of Participation. 
c - Makes and documents appropriate changes to discharge plan when necessary. 
d - Proactively uncovers barriers to early/timely discharge and overcomes them.

A Hospital ER is seeking an Emergency Medicine Physician for locum tenens coverage. Springfield, Massachusetts is home to a 24/7 casino and entertainment complex for you to enjoy while off-duty! There are also all kinds of authentic Greek and BBQ food all around the city. On top of that there are plenty of museums and parks to enjoy your time off as well. We are seeking a Board Certified (ABEM or AOBEM) or Board Eligible emergency physician with at least 5 years of residency experience. The position offers 8-10 shifts per month, including both day and night shifts, with 12-hour shifts. The facility sees an annual volume of 55,000 patients and provides support from MDs and APPs. The EMR system used is Epic, and the ED has a Fast Track system. Hospitalists manage admissions.
