This full-time position offers the opportunity to work remotely while facilitating essential interactions with clients and families through virtual means and in-person visits as required. The community center operates primarily Monday through Friday from 8:00 AM to 4:30 PM; however, occasional evening or weekend hours may be necessary to accommodate client needs. Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental, and social factors.

• Collaborate with clients and their families to devise tailored care plans that address individual goals and preferences.

• Provide counseling and emotional support, offering guidance through life transitions, health challenges, and other personal issues.

• Act as an advocate for clients' rights and needs, helping them access community resources and ensuring they receive essential services.
