This full-time position offers the opportunity to work remotely while facilitating essential interactions with clients and families through virtual means and in-person visits as required. The community center operates primarily Monday through Friday from 8:00 AM to 4:30 PM; however, occasional evening or weekend hours may be necessary to accommodate client needs. Flexibility in your schedule is essential to ensure effective support and continuous care for our clients.
Key Responsibilities\n

- Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, mental, and social factors.
- Collaborate with clients and their families to devise tailored care plans that address individual goals and preferences.
- Provide counseling and emotional support, offering guidance through life transitions, health challenges, and other personal issues.
- Act as an advocate for clients' rights and needs, helping them access community resources and ensuring they receive essential services.
- Coordinate care with an interdisciplinary team consisting of nurses, caregivers, and healthcare professionals to optimize client outcomes.
- Perform regular client visits to monitor well-being, assess living conditions, and address safety concerns.
- Facilitate communication among family members involved in the senior client’s care, aiding in conflict resolution when necessary.
- Educate clients and families on available community resources, support groups, and programs to improve social engagement and quality of life.
- Maintain meticulous documentation of client assessments, care plans, progress notes, and interactions according to regulatory standards.

Compensation and Benefits\n
