Job DescriptionLicensed Social Worker (MSW) - Social Services ManagerFull-Time | Up to $93,000 per year | Comprehensive BenefitsWe're seeking a Licensed Social Worker (MSW required) to join our leadership team as a full-time Social Services Manager. In this role, you'll oversee the Social Services Department while helping residents and families navigate admissions, care planning, and successful discharge transitions.As an important member of our interdisciplinary team, you'll collaborate with nursing, rehabilitation, and administration to ensure residents receive compassionate, person-centered care while maintaining compliance with state and federal regulations.Responsibilities include:Lead the Social Services Department and advocate for residents' psychosocial well-being.Coordinate admissions, discharge planning, and interdisciplinary care planning.Complete social services assessments, including BIMS, PHQ-9, trauma screenings, and other required documentation.Communicate with residents, families, physicians, and the interdisciplinary team to ensure continuity of care.Maintain accurate documentation in PointClickCare.Support regulatory compliance and survey readiness activities.Qualifications:Master's degree in Social Work or a related field required.Active Social Work license required (LICSW, LCSW, or LSW).Experience in social work, admissions, and/or discharge planning in a skilled nursing facility or other long-term care setting required.Knowledge of the Medicaid application process required.Strong communication, organizational, and leadership skills.We offer a competitive salary of up to $93,000 per year, based on experience, along with a comprehensive benefits package that includes:Medical insuranceDental insuranceVision insurance401(k)Paid time offSupportive leadership and collaborative team environmentOpportunity to make a meaningful difference in the lives of residents and their familiesIf you're a compassionate, licensed social worker looking for a leadership opportunity in long-term care, we'd love to hear from you.
