Work Environment\n
As a remote worker based out of Springfield, Massachusetts, you will have the flexibility to manage your schedule while maintaining a structured workweek, operating Monday through Friday from 9:00 AM to 5:00 PM. Key Responsibilities\n
In your role as a Clinical Social Worker, you will undertake a variety of responsibilities focused on ensuring holistic support for our clients. Your day-to-day tasks will include:

- Acting as a liaison between patients, families, and outpatient care providers to coordinate comprehensive care plans.
- Engaging with insurance companies to facilitate pre-certification and manage utilization reviews to ensure access to necessary services.
- Participating in the discharge planning and aftercare processes, ensuring clients transition smoothly from hospice care.
- Providing therapeutic meal support and modeling healthy relationships with food, assisting clients in achieving a balanced, nutritious lifestyle.
- Delivering empathetic and affirming support by understanding and honoring each client’s unique identities and backgrounds, ultimately enhancing their wellbeing.

Patient Population / Client Focus\n
This role primarily focuses on providing support to older adults facing end-of-life challenges, alongside their families and caregivers. Qualifications\n
To be successful in this role, candidates must meet the following qualifications:

- A Master’s degree in Social Work from an accredited institution.
- A valid LICSW license in the state of Massachusetts.
