Key Responsibilities\n

- Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and financial needs, involving family members when necessary.
- Provide individual and group counseling to patients dealing with grief, adjustment issues, and other mental health challenges.
- Work closely with medical and interdisciplinary teams to develop and implement comprehensive care plans, ensuring that psychosocial concerns are integrated into overall treatment strategies.
- Coordinate with community resources to secure post-discharge services for patients, facilitating their smooth transition from inpatient care to home care or outpatient services.
- Offer education and support to patients and families about treatment options, healthcare navigation, and available resources for emotional and financial assistance.
- Document patient interactions accurately and timely, adhering to legal and ethical standards in compliance with hospital policies.
- Engage in crisis intervention when necessary, providing immediate support to patients and families in crisis situations.
- Participate in institutional committees, contributing your expertise in social work to enhance hospital policies and practices.

Compensation and Benefits\n
