Pay Range: $35.00 - $41.00 per hour 32-35 hours per week
Working under the general supervision of the Facility Administrator and/or Director of Nursing, the Social Worker is responsible for coordinating resident admissions, advocating for resident needs, and ensuring appropriate social and emotional support services are provided throughout the resident's stay.

Responsibilities
- Coordinate and participate in resident and family activities that promote social engagement, reality orientation, and intellectual stimulation.
- Assess resident and family situations and identify those involved in the resident's care planning process.
- Provide counseling and emotional support to residents and families regarding illness, adjustment, loss, grief, and end-of-life concerns.
- Ensure compliance with all applicable federal and state regulations governing social services in long-term care.
- Document resident social and emotional needs and develop appropriate plans of action.
- Refer residents to social, healthcare, and community agencies while maintaining complete referral documentation.
- Serve as a liaison between residents, families, healthcare providers, and community organizations.
- Assist residents and families in identifying community and financial resources.
- Plan, schedule, and facilitate room visits and individualized activities for residents unable to participate in group programs.
- Coordinate activities that support residents' psychosocial well-being.
- Collaborate with the Director of Nursing in the completion of MDS documentation.
- Participate on interdisciplinary teams and facility committees as a resident advocate.
- Follow all company policies, procedures, and safety regulations.
Qualifications
- Bachelor's degree in Social Work or a related field required.
- Massachusetts Social Worker License (LSW) required.
- Minimum of 3 years of Social Work experience.
- Experience in skilled nursing, rehabilitation, long-term care, or healthcare settings preferred.
- Strong knowledge of federal and state regulations affecting long-term care residents.
- Excellent communication, counseling, and interpersonal skills.
- Ability to build strong relationships with residents, families, physicians, and interdisciplinary team members.
