Position Summary

We’re seeking an experienced and clinically knowledgeable Hospital Liaison/Screener to join the team at The Knollwood Nursing Center within the Briarwood Community in Worcester, MA.

In this role, you’ll evaluate prospective patients at area hospitals for admission to Knollwood’s sub-acute unit. You’ll assess clinical appropriateness, available resources, care needs, and reimbursement considerations to help ensure appropriate placement and high-quality outcomes.

You’ll work closely with hospital teams, physicians, HMOs, nursing leadership, therapy staff, residents, and families while representing Knollwood throughout the referral and admission process. This position also plays an important role in developing relationships with area hospitals and rehabilitation centers to promote Knollwood’s sub-acute services.

Position Type: Part-time

Hours: Every other weekend 8am-4:30pm

Pay Range: $30-$40 an hour

Your Role

Working closely with the Nursing Center Administrator, Unit Manager, Therapy Manager, hospital partners, and other members of the care team, you will:

• Evaluate patients at area hospitals for admission to the sub-acute unit based on clinical appropriateness and Knollwood’s available resources
• Assess patient needs and care requirements to support appropriate placement and high-quality outcomes
• Review patients in relation to HMO contracts and reimbursement considerations to help ensure that reimbursement appropriately supports the cost of care
• Consult with nursing and therapy leadership to determine appropriate placement and admission suitability
• Complete required clinical and administrative documentation to support the admission process
• Coordinate and communicate with hospital staff, physicians, families, HMOs, and other healthcare professionals throughout the referral process
• Represent Knollwood at hospital and HMO meetings related to sub-acute policies, procedures, and services
• Build and maintain professional relationships with area hospitals, rehabilitation centers, HMOs, and referral sources
• Conduct marketing and outreach activities with area hospitals and rehabilitation centers to promote Knollwood’s sub-acute services
• Communicate evaluation findings and recommendations clearly and effectively, both verbally and in writing
• Apply knowledge of medical conditions, pharmacy needs, rehabilitation services, and care requirements when evaluating prospective admissions
• Work independently while effectively organizing priorities and managing responsibilities across multiple referral sources
• Perform other duties and responsibilities as assigned

Qualifications

• Current RN, LPN, LICSW, or LCSW credentials
• At least one year of experience in long-term care and hospital/geriatric services
• Experience with patient interviewing, assessment, and evaluation
• Working knowledge of care considerations associated with a wide range of illnesses and disease conditions
• Understanding of pharmacy and rehabilitation implications related to patient care and reimbursement
• Strong interpersonal and communication skills with the ability to interact effectively with residents, families, visitors, physicians, coworkers, hospital personnel, HMOs, and other healthcare professionals
• Proficiency with computers and standard word-processing applications, including Microsoft Word
• Strong organizational and time-management skills
• Ability to plan, prioritize, and work independently
• Ability to clearly articulate evaluation results and recommendations verbally and in writing
• Professional judgment and the ability to assess clinical appropriateness and available resources
• Ability to travel between Knollwood and area hospitals and rehabilitation centers
• Ability to lift less than 20 pounds
• Must also be to complete the mandated PASRR form on every admission

Work Environment

This position operates in both a professional office environment and public healthcare settings, including area hospitals and rehabilitation centers.

The role routinely involves the use of standard office equipment, including computers, telephones, photocopiers, filing systems, and fax machines.

The Hospital Liaison/Screener must be able to:

• Travel between the primary work site and area hospitals and rehabilitation centers
• Communicate effectively in English, including hearing and speaking
• Use a computer and standard office equipment
• Lift less than 20 pounds
