Andover Forest Post Acute Care Center is hiring a Licensed Social Worker for a full-time Social Services Manager role in a skilled nursing facility.
Open to both BSW and MSW candidates with active license.
Pay:

BSW: $65K-$75K/year
</li>

MSW: up to $93K/year
</li>
</ul>
What You'll Do:
<ul>
<li>
Lead Social Services program
</li>
<li>
Manage admissions and discharge planning
</li>
<li>
Complete assessments (BIMS, PHQ-9)
</li>
<li>
Participate in care plan meetings
</li>
<li>
Work closely with nursing and IDT
</li>
<li>
Support residents and families
</li>
</ul>
Requirements:
<ul>
<li>
Active Social Work license
</li>
<li>
BSW or MSW
</li>
<li>
Experience in skilled nursing or long-term care preferred
</li>
</ul>
