Key Responsibilities\n

- Conduct in-depth psychosocial assessments through interviews, observations, and thorough reviews of medical histories to gain an understanding of the emotional and social dynamics influencing the patient’s well-being.
- Deliver empathetic counseling and support to both patients and their families, guiding them through adjustment challenges, grief, and emotional distress associated with terminal illness.
- Collaborate with a multidisciplinary team that includes physicians, nurses, and chaplains in developing individualized treatment plans that holistically address the psychosocial needs of patients.
- Facilitate referrals to community resources, encompassing support groups, respite care, and vocational rehabilitation, ensuring patients have access to comprehensive support systems.
- Provide educational sessions for patients and families to enhance understanding of available community resources and the available post-rehabilitation care options.
- Engage in discharge planning, working alongside the medical team to ensure smooth transitions for patients moving to home care or alternative care settings.

Compensation and Benefits\n
