Job Title: Social Worker – LICSW (Travel Contract)
Location: Hyannis, MA 02601
Duration: 26 Weeks
Schedule: 7:30 AM – 4:00 PM | Rotating days, weekends & occasional holidays
Job Type: Contract / Travel
Job Summary:
"Navitas Healthcare, LLC" is seeking a Licensed Independent Clinical Social Worker (LICSW) to provide comprehensive psychosocial support to patients and families in a healthcare setting. This role focuses on assessment, counseling, discharge planning, and care coordination to ensure continuity and quality of care.
Key Responsibilities:
- Conduct psychosocial assessments for patients and families
- Develop and manage individualized care and discharge plans
- Provide counseling and emotional support
- Collaborate with interdisciplinary healthcare teams
- Support crisis intervention and resource planning
- Coordinate referrals to community resources and services
- Advocate for patient rights and access to care
- Maintain accurate and timely documentation
- Ensure compliance with clinical and regulatory standards
- Educate patients and families on coping strategies and care plans
Qualifications:
- Minimum 2 years of clinical social work experience in a healthcare setting
- Experience with discharge planning and crisis intervention
- Strong communication and interpersonal skills
- Acute care or hospital experience preferred
Education & Certification Requirements:
- Master’s Degree in Social Work (MSW) from an accredited program
- Active LICSW license in Massachusetts
- BLS certification preferred
