Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.

UVS is seeking a Specialized Service Coordinator candidate who is:

UVS is seeking a Specialized Service Coordinator to provide and coordinate support services for individuals with developmental disabilities and mental illness, requiring specialized care.


We are offering a generous sign on bonus!!$500 at time of hire and another $500 upon successfully completing six months at UVS


Thorough initial and ongoing training is provided!


Specialized Service Coordinator Responsibilities include:


Consulting with other service providers and resource staff regarding the assessment, treatment, andrehabilitation needs of the individuals.
Coordinate and monitor all services provided to individuals of assigned caseload
Ensure all components of the Health and Wellness Guidelines of the Agency and the Division ofDevelopmental Services are applied.
Helping individuals to remain responsible for their own lives by collaborating with them in
developing their individual plan of care, help them maintain the skills they need to live in the
community, maintain or obtain housing, and income sources as needed.
Keep informed regarding Agency, regulatory changes, and legislation affecting individuals withintellectual and developmental disabilities.
Maintain complete and accurate case records and reports and submit documents on a
regular/timely basis.
Monitor, reassess and change individuals care plan as needed and approved by the individual perthe Agency and Developmental Services policies and protocols.
Monitoring individuals progress and response to medications.
Oversee respite services provided to families or Shared Living Providers (i.e., administration,
communication, support, etc.) for assigned caseload.
Participate in appropriate Agency, community, and State-wide committees.
Serve as the primary resource and liaison to Shared Living Providers requiring regular home
visits.
Supervise residential programs and subsequently employed staff as assigned.
Coordinate supportive counseling, medication education, mental health education and
early crisis intervention for individuals of assigned caseload when required.
Transport individuals to appointments or other activities as necessary for service coordination
and/or program requirements.
Provide coverage for staff shifts as needed.
