POSITION SUMMARY

A respected healthcare facility in Fall River, MA is seeking a dedicated and compassionate Licensed Social Worker to join its care team.

The Social Worker will provide essential psychosocial support to residents and their families while helping address social, emotional, and behavioral needs. This position plays an important role in resident advocacy, care planning, family communication, and coordination of services.

The ideal candidate will have prior experience in skilled nursing, long-term care, or another healthcare environment and be committed to providing respectful, person-centered care.

KEY RESPONSIBILITIES

  • Provide social work services and psychosocial support to residents and their families.

  • Evaluate residents’ social, emotional, and psychosocial needs and develop appropriate care interventions.

  • Advocate for resident rights, dignity, independence, and overall quality of life.

  • Participate in interdisciplinary care planning meetings and resident conferences.

  • Help residents and families access community resources, referrals, and support programs.

  • Provide counseling, emotional support, and appropriate psychosocial interventions.

  • Build effective relationships with residents, families, staff, healthcare professionals, and community organizations.

  • Coordinate with nursing, rehabilitation, activities, administration, and other departments to support comprehensive resident care.

  • Assist residents and families with transitions, changes in health status, and adjustments to care needs.

  • Maintain accurate and confidential social service records and documentation.

  • Address behavioral health and psychosocial concerns through appropriate resources and referrals.

  • Participate in facility quality improvement programs and initiatives.

  • Promote a welcoming, respectful, and supportive environment for residents and their families.

QUALIFICATIONS

  • Bachelor’s degree in Social Work, Social Services, or a related discipline required.

  • Current and valid Massachusetts Social Worker license required.

  • Prior experience in skilled nursing, long-term care, nursing home, or healthcare settings required.

  • Understanding of psychosocial services, behavioral health, resident care, and long-term care regulations.

  • Strong communication, interpersonal, organizational, and problem-solving abilities.

  • Excellent advocacy and family-support skills.

  • Compassionate, professional, and resident-centered approach.

  • Ability to collaborate effectively within an interdisciplinary healthcare team.

  • Strong documentation and administrative skills.

  • Must successfully complete required background and employment checks.
