Key Responsibilities\n

- Assist patients in navigating community resources, which may include connecting them to support groups, vocational rehabilitation, or housing assistance to enhance their recovery journey.

- Play an active role in discharge planning, facilitating smooth transitions to home or other care settings with appropriate follow-up support.
