This position offers a fully remote work setting, providing flexibility while allowing you to serve our client population effectively. While primarily focused on a Monday to Friday schedule from 8:00 AM to 4:30 PM, we recognize the importance of adaptability. There may be occasional evening or weekend hours required based on the needs of our clients, and you should be prepared for possible on-call duties to address urgent matters as they arise.
Key Responsibilities\n

- Conduct comprehensive assessments of clients’ psychosocial needs, taking into account their emotional, mental, and social circumstances.
- Collaborate closely with clients and their families to formulate personalized care plans that align with their specific goals and preferences.
- Provide compassionate counseling and emotional support, assisting clients in navigating life transitions and addressing challenges related to addiction and recovery.
- Advocate for clients’ rights and help them access necessary community resources, ensuring they receive the appropriate support and services available.
- Coordinate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to enhance overall client care and service delivery.
- Conduct regular check-ins with clients to monitor their well-being, evaluate their living conditions, and address any safety concerns.

- Maintain accurate and timely documentation of client assessments, care plans, progress notes, and other pertinent interactions using electronic health record (EHR) systems.
