Job Title: Licensed Clinical Social Worker (LCSW)
Location: Hyannis, MA 02601
Job Type: Contract (26 Weeks, with possibility of extension)
Schedule:
- Shift: Day Shift (8:00 AM – 4:00 PM)
- Hours: 40 hours per week
- Rotating days and weekends
Job Summary:
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide comprehensive social work services in a healthcare setting. The ideal candidate will support patients and families through psychosocial assessments, care coordination, discharge planning, and connection to community resources.
Responsibilities:
- Conduct psychosocial and risk assessments, including mental health, substance use, family, and environmental factors
- Develop and implement discharge and aftercare plans in collaboration with the care team
- Provide counseling, crisis intervention, and emotional support to patients and families
- Coordinate with healthcare providers and community organizations to ensure continuity of care
- Assist patients with access to services, medical equipment, and long-term care placement
- Maintain accurate and timely documentation in accordance with policies and ethical standards
Requirements:
- Master’s degree in Social Work (MSW)
- Active LCSW or LICSW license required
- Strong communication, organizational, and problem-solving skills
- Ability to work effectively in a fast-paced, high-pressure environment
- Basic computer proficiency
- CPI/MAB certification (or willingness to obtain)
Preferred Qualifications:
- Prior hospital or healthcare setting experience
