Job Description
This direct support position assists individuals with intellectual and developmental disabilities in building and maintaining their ability to participate in community activities by focusing on skills in communication, self-care, relationship-building, and community involvement. As a Direct Support Professional / Community-Based Day Supports Coordinator – Level I, you will work in concert with each assigned program participant and AGB Team members to develop and implement their Person Centered Plan (PCP) and their Individual Support Plan (ISP).
