Registered Nurse – LTC / Health Home Management (Assessment RN)
Schedule: 5x8 Days (8:00 AM–4:00 PM / 9:00 AM–5:00 PM, flexibility required)
Guaranteed: 40 Hours/Week

We are seeking an RN with Home Health, LTC, or similar experience for a Health Home Management role focused on patient assessments. This is a non-bedside position with no hands-on patient care.

Requirements:
\n
- \n
- Active Massachusetts RN License
- BLS Certification
- Minimum 1 year of recent RN experience
- Experience with MDS or OASIS assessments (required)
- Reliable transportation
- Willing to float to all CHL locations (up to 38-mile radius)

Responsibilities:
\n
- \n
- Complete patient assessments in the home and/or office
- Develop and update care plans
- Medication reconciliation
- Work alongside a care partner who knows the patient
- Documentation for insurance/program requirements
\n

\n
\n
Notes:

- \n
- Home Health, LTC, or similar assessment experience accepted
- Psych experience is a plus, not required
