The Grove at Carvalho is seeking a compassionate and dedicated Licensed Social Worker (LSW) to join our healthcare team in Fall River, MA.
The LSW will provide social work services to patients and residents while collaborating with families, caregivers, nursing staff, rehabilitation professionals, and the interdisciplinary care team. This position supports the emotional well-being, dignity, quality of life, and overall care needs of our residents.
Responsibilities

Provide social work services based on individual patient and resident needs.

Conduct psychosocial assessments and identify social, emotional, and environmental needs.

Develop and implement individualized interventions and care plans.

Provide emotional support, counseling, and crisis intervention as appropriate.

Support patients and families as they adjust to illness, disability, changes in functional status, and long-term care.

Advocate for patient rights, preferences, dignity, and quality of life.

Assist with discharge planning and transitions of care.

Connect patients and families with community resources, benefits, and support services.

Collaborate with nursing, rehabilitation, medical, and interdisciplinary team members.

Participate in care planning meetings and interdisciplinary conferences.

Address patient and family concerns in a professional and timely manner.

Maintain accurate and timely documentation.

Assist with advance directives, guardianship matters, and other psychosocial needs as appropriate.

Participate in quality improvement initiatives and facility programs.

Maintain patient confidentiality and follow all applicable regulations, policies, and procedures.
Qualifications
-
Current and valid Massachusetts Licensed Social Worker (LSW) license.

Bachelor’s or Master’s degree in Social Work from an accredited program, as required by applicable regulations.

Previous experience in skilled nursing, long-term care, rehabilitation, healthcare, or a related setting preferred.

Strong communication, interpersonal, organizational, and documentation skills.

Ability to work effectively with patients, families, caregivers, and interdisciplinary healthcare teams.

Strong problem-solving and critical-thinking skills.

Ability to handle sensitive situations with professionalism and empathy.

Must be able to pass required background and employment checks.
