Our 46 bed skilled nursing and rehabilitation facility is looking for a professional Licensed Social Worker to bring their passion, skills and love of the geriatric population to our community.

As the Director of Social Work Services, you are an integral part of the interdisciplinary care team and will interface with residents and family members alike ensuring a smooth transition into our community, assisting throughout their stay and when indicated assisting in the discharge planning process.
-Previous SNF experience preferred, but will train
-Knowledge of MDS/PCC is beneficial but not required. Will train
- Participates on the Admission Team, with the Administrator and Director of Nursing, to assess the appropriateness of referrals for admission to the facility.
- Overall integration into the SNF/LTC environment and facilitates family meetings for discharge planning/transition into LTC environment
- Completes appropriate assessments- Will train as needed
- Possess a social work license in Massachusetts
