Part-Time Opportunity with Carease Health - MA

At Carease Health - MA, we're dedicated to providing comprehensive preventative care programs that prioritize the overall well-being of our patients. As a Care Companion/Social Worker, you'll play a vital role in our Collaborative Care team, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About the Role:

We're seeking a compassionate and dedicated Mental Health Provider to join our team. In this part-time role, you'll have the opportunity to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment. As a Care Companion/Social Worker, you'll be responsible for:

• Visiting designated facilities in accordance with your scheduled visits with your supervisor
• Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Documenting visit notes and maintaining detailed records of interactions and interventions after each visit
• Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Requirements:

• Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year
