Skilled Nursing Facility | Up to 13 Weeks
We're seeking an experienced Director of Social Services for a temporary (up to 13-week) assignment - with strong potential to transition into a permanent, full-time role.
Highlights:

Oversee social work, admissions & discharge planning

Complete assessments (BIMS, PHQ-9, trauma screening)

Coordinate aftercare, care plans & family communication
Requirements:

Master's in Social Services or related field

Active LICSW, LCSW, or LSW

SNF/long-term care & Medicaid experience
$50/hour | Direct hire - not an agency role
