Responsibilities

<li>Ensure the confidentiality of all medical records information and complete forms for release of information, referring any questionable requests to the Medical Records Supervisor/Health Services Administrator (HSA). </li>

By coordinating with Re-Entry Services and Clinical Leaders, you'll ensure continuity of care and optimal patient outcomes in the community setting.
 
Responsibilities

<li>Collaborate with DOC Re-entry services for discharge planning, ensuring adherence to policies and procedures for patients with mental health issues or serious medical illnesses.</li>
<li>Review requests from external agencies regarding medical and mental health transfers back into DOC institutions, determining appropriateness of transfer and coordinating necessary services.</li>
<li>Maintain relationships with community agencies and communicate with Medicaid Coordinator to ensure resumption of benefits for released patients.</li>
<li>Advocate for medical necessity and continuation of prescribed medications for patients admitted to community facilities, while attending relevant meetings and participating in staff training.</li>
<li>Consult with supervisory personnel and appropriate leadership regarding complex cases, identify areas of concern, and facilitate resolution while maintaining security clearance and applying critical thinking skills to varied situations.</li>

Responsibilities
About the Role

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: 

<ul>
<li>Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Administer and order point of care and lab-based testing as needed</li>
<li>Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li>
<li>Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities</li>
</ul>

Those completing a monthly average of 20 – 100 visits earn $1,800 – $11,500 per month. Qualifications
Skills &amp; Experience That Shine

<li>Master’s Degree or commensurate experience and satisfactory completion of NP licensure</li>
<li>Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed</li>
<li>AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty</li>
<li>Current BLS, ACLS or CPR certification</li>
<li>1 year of NP experience preferred, new grads encouraged to apply!</li>
<li>Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence</li>
