Purpose Statement
Home infusion therapy (HIT) is a method of administering infusion therapy directly in a patient's home as well as in other settings such as nursing homes, hospitals, and outpatient clinics. The versatility of HIT provides patients with more autonomy over their care.
The role of a Home Infusion RN involves delivering excellent patient care. This specialized position caters to adult patients, providing them with the necessary treatment. As a Home Infusion RN, your primary responsibility is infusion therapy involving a variety of treatments including but not limited to IVIG, monoclonal antibodies, enzyme replacement, clotting factor, and other agents using various methods of administration. This job requires a blend of clinical expertise and compassionate care.
This position reports to the Area Director of Infusion Services.
Required Education and Experience

SUMMARY:
The Staff Nurse, RN or LPN will have knowledge and understanding of substance abuse and mental health illness and the capability to provide treatment consistent with standards of professional care. This position requires the physical ability to perform emergency treatment, e.g. CPR, assisting patients with routine care, etc. The RN or LPN will conduct a medical assessment, intake screening exams, and have the capability to implement physician’s standing orders.

ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:
<ul>
<li>Perform pre-admission screenings (telephone and on-site), for patients requesting admission.</li>
<li>Maintain patient charts, including nurse’s notes and med sheets, physician order forms, assessments and other essential documentation in a clear, concise and comprehensive manner.</li>
<li>Perform physical assessments on patients admitted to a Gosnold facility in accordance with standards.</li>
<li>Monitor drug and alcohol withdrawal to prevent development of medical complications and administer medications in accordance with physician orders.</li>
<li>Evaluate and document identified medical problems and report them in a timely manner to the Medical Director or the on-call staff physician.</li>
<li>Adhere to Universal Precautions for Infection Control and to proper procedure for disposal of sharps.</li>
<li>Maintain nurse’s station neat, free of hazard, and consistent with health, safety, and sanitation standards.</li>
<li>Participate in case reviews and in-service training programs.</li>
<li>Maintain awareness and working knowledge of all Gosnold procedures relevant to this position.</li>
<li>Adhere to the Professional Staff Code of Ethics.</li>
<li>Conduct patient education sessions on issues of addiction, health maintenance and related recovery topics.</li>
<li>Perform all other duties as assigned.</li>

 MINIMUM KNOWLEDGE, EXPERIENCE, AND SKILLS REQUIRED:

Education:

<li>Certificate, Associate’s or Bachelor’s Degree in Nursing. </li>

Experience:

<li>One year of experience in nursing is preferred. </li>

Certificates, Licenses, and Registrations:

Job Description
North Suffolk is looking for Outreach Specialists to join our team! As an Outreach Specialist - Community Support Specialist you will participate in the engagement, assessment, and treatment planning process for persons served. You will work with a dedicated team of staff to address the needs and goals of the persons served in both community and group living environments. The team provides person-centered interventions and treatment plans in the areas of housing, risk mitigation, transportation, skill-building, money management, and medication administration to support persons served to live a self-directed life.
Salary starting at: 51,300 yr 
What you'll do:
<ul>
<li>Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, and develop and/or restore skills impacted by mental illness.</li>
<li>Gather information about community agencies to assist in the provision of services.</li>
<li>Provide side-by-side support and coaching to help people socialize.</li>
<li>Organize and lead individual and group social and recreational activities.</li>
<li>Carry out the assigned interventions and action steps of the treatment plan, through the provision of direct support and rehabilitation services to people on an individual, group, and family basis (in conjunction with professional staff) in the office and in community settings.</li>
<li>Collaborate with Housing Coordinators to help each person in finding a safe and affordable place to live – apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating.</li>
<li>Participate in medication administration per MAP and DMH policies and protocol.</li>

What you’ll need:

<li>High School Diploma/GED equivalent required. </li>
<li>Bachelors Degree highly preferred</li>

<li>In lieu of college degree, at least one year experience with population that has severe and persistent mental illness and/or substance use disorder</li>
<li>Bilingual Spanish and American Sign Language skills desired but not required - please indicate language capability in application</li>

<li>Must be MAP and CPR/First Aid certified within five months of hire (training provided) </li>
