Residential specialists are tasked with providing support around daily living and mental health management in a 24/7, community-based adult residence. He enjoys going into the community and visits a variety of community locations including Upreach in Goffstown, NH.

General Responsibilities:
· Provide skilled shift coverage, including weekends and awake overnights, as scheduled by the supervisor, Program Manager or Program Director
· Implement treatment programs, including Individual Service Plans (ISPs) and clinical programs ; collect behavior and academic data; complete log entries
· Teach daily living skills such as safety rules, travel-training, banking and budget management, cooking, shopping, personal care and hygiene, and care of personal space and belongings
· Provide supportive counseling to clients concerning daily, personal and social issues

Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness, co-occurring developmental disabilities and substance use disorders. Assertive Community Treatment (ACT) is an evidenced based practice model that provides an intensive and highly integrated approach to community mental health services. ACT support helps people with severe and persistent mental illness become independent and live in the community as they work toward their recovery goals. ACT support also helps with reducing homelessness and unnecessary hospital stays. 
 
The Adult ACT Medication Training and Support (MTS) Specialist provides outpatient community-based counseling support to educate; problem solve and develop strategies to incorporate prescribed medication into daily routine and lifestyle. Additionally, they will assist clients with improving mental and physical health symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a per diem position - hours vary based on need.

Becca is a passionate, playful, and genuine 31- year-old woman. She loves her animals, and they sometimes help regulate her anxiety at home and in the community. When at a behavioral baseline, Becca enjoys social interactions with others, especially those close to her, and with whom she has formed relationships within her social circles. She does exhibit intense behavioral outbursts. Becca has a close relationship with her family and likes to socialize with friends whom she has maintained relationships with from Special Olympics'. Becca also attends equine therapy where she helps care for the horses and enjoys going out to eat and shopping.
She is mobile but requires a wheelchair for long distances as she tires easily and is at high risk for drop seizures. She also utilizes a gait belt/harness for walking and transfers, as she has an extensive seizure history. Behavioral challenges along with ongoing seizure activity continue to be an issue for Becca's day-to-day living and functioning. She requires personal care support from her home care provider and staff but has been working on her independence with her continence and hygiene. Becca uses some verbal communication and picture exchange communication (PECS) and visual board communication but requires support in communicating.

<li>Experience with Alzheimer's, Dementia, and Hospice care preferred but not required&nbsp;&nbsp;</li>
<li>Criminal Background Record Check</li>
<li>A minimum of 4&nbsp;references (2 personal and 2 professional)</li>
<li>Recent TB test completed within the past year</li>
<li>Working every other weekend is a requirement</li>
