Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Collaborative Care Program is an innovative approach to healthcare that encompasses the integration of mental, physical and behavioral health needs. Working together as a multi-disciplinary team, internal (CLM) and external (area agencies) work as one entity to support individuals with co-occurring mental health needs and intellectual disabilities and/or traumatic brain injury. The Collaborative Care Community Support Counselor provides outpatient community-based support to assist client’s with improving symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full-time, 35 hour per week position. 
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide psychoeducation using the evidence-based practice of Illness Management and Recovery (IMR) to support goal and coping skill development. </li>
<li>Provide counseling and Functional Support Services (FSS) in the home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.</li>
<li>Provide Family Training and Counseling (FTC) services to educate natural supports and promote how to best encourage the client’s resiliency and recovery toward treatment goals.</li>
<li>Provide Medication Training and Support (MTS) services, as needed to educate, problem solve and develop strategies to incorporate prescribed medication into daily routine.</li>
<li>Provide Crisis Intervention (CI) services, as needed, when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. </li>
<li>Adhere to and uphold the practices specific to the Continuum of Collaborative Care Model specific to support the authenticity of program implementation. </li>
<li>Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care.</li>
<li>Provide timely documentation on each service delivered.</li>
<li>Provide set hours of billable service each week (eligible for team-based incentive pay upon exceeding service expectation). </li>
<li>Engage in appropriate consultation meetings and committees, both internally and externally, as determined by supervisor.</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients.</li>
<li>Execute all required services and documentation in accordance with agency policies and federal/state standards.</li>
<li>Maintain professional memberships, licenses, accreditation, certifications, etc.</li>
<li>Represent CLM in a professional and appropriate manner in all settings.</li>

<li>Assist client with personal care and hygiene</li>
<li>Plan and prepare meals with assistance from the client</li>
<li>Assist with shopping errands</li>
<li>Perform light housekeeping duties</li>
<li>Be a pleasant and supportive companion</li>
<li>Report any unusual incidents</li>
<li>Act quickly and responsibly in cases of emergences</li>

Responsibilities
<ul>
<li>Assist clients with 1 on 1 personal care and home management as instructed by and documented on the written plan of care.</li>
<li>Assist and facilitate activities of daily living (ADLs) such as personal-care and hygiene, leisure activities and socialization.</li>
<li>Help local seniors at home with shopping and/or light housekeeping, laundry, meal preparation and companionship.</li>
<li>Always ensure client safety.</li>
</ul>
Requirements
<ul>
<li>A compassionate, warm, and accepting attitude toward people.</li>
<li>Ability to respect confidentiality of patients and families.</li>
<li>Compassionate, caring, and empathetic</li>
<li>Excellent customer service skills.</li>
