Better Care Home Health, Inc. is currently seeking an experienced Physical Therapist (PT) to perform home health visits for our Leominster, MA office. This office provides services to clients throughout Central Massachusetts. As a Home Health Physical Therapist, you will be an integral member of a multidisciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home. You will be responsible for evaluating and managing the Physical Therapy needs of the patient, based on a comprehensive assessment, in line with the overall Plan of Care.
Better Care Home Health, Inc. is seeking a Part Time (24-32/week) clinician. Responsibilities include:
Making home visits to clients in designated geographic territories.
Working collaboratively with a Physical Therapy Assistant, you will provide PT to an adult and geriatric population in their homes in Central Massachusetts.
Prior home care experience is preferred but not required.
As a home care Physical Therapist you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions which support the needs of the patient.
Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided utilizing the EMR.
Qualifications for the ideal candidate:
Home care and/or hospice experience is a plus.
Demonstrated ability to read, write, and effectively communicate in English.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases, as assigned.

Requirements:
Internet-capable Computer, Smartphone and/or Tablet (iPad/Android) for accessing and working within the Electronic Medical Record (EMR).
All applicants will be required to submit a background check authorization with the Massachusetts Criminal Offender Information Registry (CORI).

A valid, unrestricted MA drivers license and reliable transportation.
Valid, unrestricted licensure as a Physical Therapist (PT)in the State of Massachusetts.
