• Skills checklist (specific to Radiologic Tech, completed within the last year)
• Two (2) supervisory references from the last two years
• Certifications — ARRT, BLS (AHA) (do not need to be active at time of submission but must be willing to obtain)
• State License: Vermont (must be eligible and willing to obtain)
