Coordinates a wide variety of therapeutic, educational, basic supports and integrative services to psychiatrically disabled children to ensure they receive the services and assistance required for successful maintenance in the least restrictive environment and for continuity of care. In addition direct intervention in the community setting is provided to enhance goal attainment. Hours vary through the week to accommodate families/clients. Qualifications: BA in a human service field; one years' experience working with SED children and their families; skills in advocacy, outreach, assessment, crisis intervention and supportive counseling; and strong communication skills. Good driving record and reliable transportation required.
Full-time positions. Keene. Adult and Older Adult Services. Plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Works independently and must demonstrate excellent clinical judgment, decision-making and safety consciousness to assure safety and appropriate treatment for consumers in the community. Works independently without regular supervision in community settings along with consumers or leading therapeutic groups. Qualifications: BA in Human Services or related field, and one-year work-related experience, skills in advocacy, outreach, assessment, crisis intervention and supportive counseling; and strong communication skills. Good driving record and reliable transportation required.
Full-time and per diem positions. Variable shifts, will include nights and weekend hours. Provide supervision of residents habilitation programs structures therapeutic milieu within the residence, training in skills of daily living, monitors medication distribution, provides case management activities, facilitates community integration and participates in the provision of crisis care to individuals with severe and persistent mental illness. Qualifications: Associate's Degree in human services field. Related work experience and good driving record required.
Clinical Intake Specialist
Full-time position. Will perform intake and adult clinical services. Performs Child and Adult Bio-psychosocial Assessment and Eligibility Determination for intake into all MFS services. Assists in planning, coordinating, and monitoring intake practices for Adult outpatient therapeutic, rehabilitative, support and community integration services. Will carry a clinical case load. May travel to satellite offices as needed. Qualifications: Masters Degree in psychology, social work, psychosocial rehabilitation or related field and is eligible for NH License (LCMHC, LICSW); and third party reimbursement. Two years of experience required.
Supported Employment Specialists
Full-time. Adult Services and Assertive Community Treatment programs. Under the direction of a written Individual Service Plan, and following a referral by a case manager, provides community employment supports to adults with a severe and persistent mental illness. These services may include the provision of mental-illness management supports in a variety of employment settings. Develops and maintains relationships with community employers and other vocational programs to increase collaborative efforts. Qualifications: A Bachelors degree in social work, rehabilitation, psychology, education, or a related human service field and one year of experience working with persons with severe and persistent mental illness or two years of experience in a human service field, preferably within supported employment services; demonstrated communication skills; ability to transport consumers when necessary to achieve treatment goals; good driving record required.
